Arizona State Power of Attorney (POA) requires one witness with valid identification and who is 18 years or older. The signer/principal must be present, have valid ID and be mentally competent. Please note, the signer must have read the documents and be competent enough to show that they clearly understand what they are signing. For long term care and hospital signings this is especially important to know!
